Equipment Rental Agreement

This is the agreement between you and Caliber Recovery & Wellness for any equipment you rent from us. You are asked to acknowledge it before payment is collected, so please read it first.

Rental term
Rentals run in weekly periods. The minimum is one week. Two- and three-week rates are published on each product page and reduce the effective weekly cost.
Your rental period begins on the date the equipment is delivered to you or collected by you, not the date you submitted your request or paid.
Extensions. Contact us by phone or text before your current period ends and we will extend weekly at the then-current rate, subject to availability. Equipment kept beyond the agreed period without an extension is charged at the weekly rate for each additional week or part week commenced.
Rentals of four weeks or more are handled as an extension or a custom quote rather than a published rate.
Payment and card on file
Payment is taken after we confirm availability and send you an itemized total covering the rental charge, delivery fee, and any applicable tax. Nothing is charged when you submit a request.
Caliber is cash-pay. We do not bill insurance and do not provide reimbursement documentation.
Card on file. We retain a payment method on file for the duration of your rental. It is used only for extensions you have agreed to, late return charges, and charges for damage or loss as set out below. We will tell you before charging it for anything other than an agreed extension.
There is no security deposit.
Permitted use
The equipment is rented to you for your own use at the delivery address you gave us. You agree that you will:
- Use it only as described in the manufacturer's instructions supplied with it
- Follow any directions given by your healthcare provider
- Not lend, sublet, or transfer it to anyone else
- Not modify, dismantle, or attempt to repair it
- Not use it outdoors or in conditions it is not designed for
- Keep it at the delivery address unless we have agreed otherwise in writing
Caliber retains ownership of all equipment at all times. Renting it gives you the right to use it for the agreed period, nothing more.
Provider orders, suitability, and contraindications
Caliber does not provide medical advice. We can explain what a device does and how to operate it. We cannot tell you whether it suits your condition, what settings to use, or how long to use it for. Those are decisions for your healthcare provider.
By renting, you confirm that you have taken appropriate advice about whether the equipment is suitable for you, and that you will use it in line with that advice.
Devices requiring a provider order. The Breg Polar Care Wave is restricted under United States federal law to sale by, or on the order of, a medical professional. You must have that order before we will confirm the rental, and we may ask to see it.
Devices requiring professional guidance. SmartCuffs 4.0 is a blood flow restriction system. It should be used under the direction of a physical therapist, physician, or other qualified professional.
General wellness devices. The Kineon MOVE+ Pro and Hooga ULTRA1500 are offered as general wellness products. Caliber makes no claim that they are FDA-cleared or FDA-approved medical devices, and makes no claim about treatment outcomes for any condition.
Weight limits. The BlessReach knee scooter has a maximum user weight of 300 lb. Do not exceed it. Whether you may bear weight through a limb is your provider's decision, not ours.
Care, damage, and loss
You are responsible for the equipment from delivery until we collect it.
Normal wear is on us. Scuffs, fading, and parts wearing out through ordinary use are expected and are not charged to you.
Charges apply for damage beyond normal use, including:
- Damage from dropping, impact, or misuse
- Water or liquid damage beyond what the device is designed for
- Damage from unauthorized repair or modification
- Missing accessories, cables, controllers, pads, or cases
- Soiling that cannot be removed by our normal cleaning process
Where equipment is damaged, we charge the actual cost of repair, including parts and labour. Where equipment is damaged beyond economic repair, lost, stolen, or not returned, we charge the replacement value of the item. The replacement value for each device is stated on your rental confirmation, so you know the figure before you pay, not after.
Tell us as soon as anything happens. A fault reported promptly is usually a swap. A fault hidden until collection is usually a charge.
Return and collection
Contact us before your period ends to schedule collection, or to extend.
Equipment must be returned with all accessories, cables, and cases supplied. Please wipe it down before collection; full sanitization is our job, not yours.
If nobody is present at the agreed collection time, we will contact you and reschedule. A $25 fee applies to a second missed collection.
Equipment not returned or made available for collection after the rental period ends continues to be charged weekly until it is recovered.
Assumption of risk and liability
Recovery and rehabilitation equipment carries inherent risks. By renting, you acknowledge that you use the equipment at your own risk and in accordance with your provider's directions and the manufacturer's instructions.
Caliber is responsible for supplying equipment that is clean, function-tested, and in working order. Caliber is not responsible for outcomes arising from how the equipment is used, from use contrary to instructions, or from conditions we were not told about.
Caliber's total liability under this agreement is limited to the amount paid for the rental, except where liability cannot be limited by law.
This agreement is governed by Colorado law and is entered into between you and Caliber Recovery & Wellness.
